First, We Discover

Before committing to a membership package that’s right for you, we begin with our Discovery Onboarding program. This 12-week process provides Dr. Bayliss and her team the opportunity to deeply understand your personal story, goals and challenges. We will gather all the relevant information and testing to get a full picture of your current circumstances. Together, we’ll create a customized treatment plan that will get you started on a path to meeting your goals.

Then, Your Membership Begins

Once onboarding is complete, you’ll transition to a 12-month membership. The package you choose will determine the number of visits with Dr. Bayliss and your health coach, offering ongoing support for your personalized wellness journey.

Hormone Management for Women

For women interested in pursuing hormone management without other medical or health concerns.

This two-step plan is designed to help women in the perimenopausal or postmenopausal stages of life achieve balance through hormone restoration, with focused and essential care. The first step is onboarding, a one-time process for new members. After onboarding, you will transition to the monthly membership plan, which can be renewed annually.

Onboarding

  • Practice initiation to include creating your patient portal, gathering paperwork and reviewing prior history and pertinent tests.

  • A 1 hour initial consultation with your MD

  • 1 DUTCH Hormone Test ($400 value discounted to $250)

  • A 30 minute follow up appointment with your MD

Monthly Membership

Once you have completed the Balance initial onboarding process you will migrate to the monthly membership program.

  • A 40 minute physician visit  for hormone management and lab test review

  • 1 DUTCH Hormone test ($400 value discounted to $250)

  • Portal messaging for quick questions and plan clarifications

  • Additional appointments for hormone related issues can be made and billed at a 20% discounted rate *

  • Additional DUTCH testing can be ordered as needed at $250 per test (discounted from $400)

Understanding our Rates

  • Does Your Office Take My Health Insurance ?
    Our practice does not bill insurance or Medicare for services rendered. Upon request our office will provide itemized receipts of your services to include CPT codes so you may submit claims independently .
  • Can I use My HSA/ FSA for Services?
    Many of our members use their FSA/HSA to pay for fees, supplements and laboratory testing. However, we recommend checking with your FSA/HSA company/carrier for more information as to how you can use these funds.
  • Why Do you Require the Discovery Onboarding ?
    Our Discovery onboarding involves more than just an introduction to our practice—it’s a comprehensive process, digging deep and wide to understand your health and your story. This thorough review enables Dr. Bayliss to better discover the root causes of your health concerns and sets the foundation for long-term success as we walk together in pursuit of transformative health and wellness.
  • How Do I know Which Plan to Choose?
    After completing the discovery process Dr. Bayliss will work with you to help you determine the plan that would be best for you.
  • What Does Secure Portal Messaging Mean?
    This communication allows for forward momentum with your treatment plan between appointments and provides ready access to Dr. Bayliss and your health coach Kaylee as needed, with greater flexibility and timely response. Messaging is ideal for quick questions and clarifications on your plan as well as updating Dr. Bayliss on your progress. If your concern is complex or a new topic, you may be advised to set up a medical appointment to ensure you are receiving the best care. Portal messaging is not for emergency care or services. If you require urgent medical attention, please call 911 or visit an urgent care center.
  • What if I Need to Cancel or Reschedule an Appointment?
    Because of the nature of Dr. Bayliss appointments, it is a significant inconvenience when appointments are canceled. If you need to cancel or reschedule an appointment, we kindly ask that you notify us as soon as possible. Cancellations made less than 48 hours before your scheduled appointment will incur a $50 late cancellation fee Rescheduled appointments must be completed within your current 12-month membership term.
  • Can I Cancel or Change My Membership?
    We do not offer refunds on our annual or monthly membership plans. You can cancel your membership at any time. However, an Early Exit Fee of $300 will apply. If you paid for your membership upfront in full, you will have access to your services through your membership year. On occasion a patient may find they are better served at a different membership level. We will give consideration to such changes on a case by case basis.
  • How Do I Get Started?
    Take your first step today by filling out our online inquiry and a member of our staff will be in touch with you within 2-3 business days. Prior to enrolling in Discovery, you can schedule a 20 minute consultation with our MD to determine if our practice is right for you. The cost of this appointment is $50.00.
  • Does Dr Bayliss provide Primary Care?
    Dr. Bayliss is a Functional Medicine Consultant and while there may be some overlap, she does not provide traditional primary care services. Nor does she provide urgent care. We ask that each member maintain or establish a relationship with a Primary Care Physician or clinic to obtain these important medical services which are outside of Dr. Bayliss’ scope of care.
  • Does Your Office Take My Health Insurance ?
